Participation Responsibilities

To activate your participation in the Nurse Preparatory Program you must:

1. Complete and submit page one of the Application. (Print, fill out and mail.)

2. If you are over 18, complete, sign and submit page 2, if you are a minor, your parent or guardian must complete, sign and submit page two.

3. Confer with your high school counselor or advisor and complete and submit a Success Plan.

4. Complete and submit your first weekly Performance Interview.

5. Carefully read all information on this web site.

To continue participation and be eligible for services you must:

1. Update and re-submit your Success Plan at the beginning of each semester.

2. Complete a Personal Performance Interview at the end of each week. (Save but don't submit)

3. Submit a Performance Interview form showing each week's responses and cumulative score at the end of each month. (Or as otherwise directed.)

4. Review the information on this web site often. Reflect upon it as you do weekly Performance Interview.

It is highly recommended that while still in High School you:

1. Take a CNA class.

2. Take a Medical Terminology class.

3. Take a Math class each year you are in High School.

4. Take an English class each year you are in High School.

Relationship of monthly stipend to educational success principles and Performance Interview:

To encourage you to study, think about, and internalize vital success principles, we have tied the monthly stipend amount directly to your score on the Performance Interviews.

We hope this will encourage you to make a habit of the academic excellence that will secure the GPA, test preparedness and other credentials needed to successfully compete for a place in Nursing School and be critical to success in a nursing program.
