Performance Interview

The performance activities are based on the principles and standards found on the previous pages of this web site.

To remain in the Program, each student must do the Performance Interview at the end of each week by pondering each question and responding with a yes or a no.

Students must keep a personal copy of each week's interview, keeping a record of score. At the end of each month they are to submit a completed Performance Interview form that shows responses and score for each of the four weeks of the month.

1. I carefully considered academic or vocational counsel or guidance this week.

2. I had perfect attendance at classes this week.

3. I had no missing assignments in classes this week.

4. I missed no quizzes, tests or exams this week.

5. I missed no Nurse Preparatory Program meetings this week.

6. I have created my Student Success Plan and I reviewed my goals and progress this week.

7. I initiated (made the first move) in a relationship action this week.

8. I made a conscious effort to concentrate and remain focused in all my classes this week.

9. On my own initiative I made an effort to increase my knowledge and skill this week.

10. I performed a self-initiated action to retain knowledge or skill this week.
